Please outline any medical or other conditions that you think we should be aware of. (If you do not inform us in advance we may not have the staffing levels or facilities ready for you – so you may not be able to shoot, even if you have booked).

Photography permission granted for use in promotions for the club (including the club facebook page)
Agree to the Terms and Conditions:
If under 18, this must be signed by parent/guardian.

Once we have received your form and confirmed your place, we will request payment of £10 by BACs

All participants must complete and sign to confirm that:
a. they will pay attention to the safety briefing on the risks involved at the start; in the event that they miss this briefing it is
their responsibility to discuss these issues with an instructor prior to starting their shooting
b. that they accept that these activities are dangerous and can result in death &/or injury; and
c. that they accept the risks involved and are responsible for their own actions &/or involvement.

Cancellation policy:
No refund will be payable for non-attendance or if no prior notice of cancellation is given.

 

Your data will be held securely during your time on the beginners course and shared only with club secretary/coaches. We may
contact you after the course for feedback purposes or for membership purposes.
